Does smoking one pipeful mean that you have to roll three times (that turn and the two after) to see if you are addicted?

No, only two times. The two turns after use. After the second roll, the risk is reduced by 1, to 0. Unless the character uses the drug again before that.

And how does the '+1 on every D6' for withdrawal affect service at the front/heroquests?

If a character has developed an addiction, all rolls, also at the front or on a heroquest, are modified by 1 per D6 to the addict's disadvantage if they don't use the drug.
